Notices for download

Please use the forms below to give notice of your application to Responsible Authorities and to the public for certain applications. Please consult the Gambling Act 2005 Regulations and our guidance notes for further details.

Compulsory advertisement

Please note that if you are applying for the grant, variation or non fast-track conversion of a Premises Licence, or for a Provisional Statement, you MUST advertise your application. 

You must also advertise your application in the local press. You can find out about the acceptable newspapers for your advertisement on this link:
