When you have completed your form, you can:
- hand the completed form in at any Council office, or
- post it to the department where you think you received a poor service (where it will be dealt with by the Service Manager), or
- post it to Chief Executive, Town Hall, Wellington Street, Woolwich, London SE18 6PW.
