Help with travel costs
Freedom pass
Disabled
Complete the application form and return it (to the address given in the form) with:
two passport photographs
written evidence of your disability and
two items showing proof of address (read on for examples).
If you think you qualify under the secondary criteria, you will need to ask your GP to complete (with stamp and signature) the medical information form.

Help with completing the forms
If you have touble downloading or printing the forms, or need help completing them, call the Mobility Team on 020 8921 2388 or visit the Council office at:

The office is open Monday to Friday between 9.30am and 5pm.
Proof of address
You can provide any two items from this list as proof of your address:
current retirement pension book
current Council Tax payment book
Council / housing association rent book
TV licence
utility bill dated within the last three months
bank / building society statement dated within the last three months
inland revenue letter dated within the last three months
Council Tax bill dated within the last three months
benefits agency letter dated within the last three months
letter of pension entitlement dated within the last three months
Disabled and under 18
If you are under 18 and disabled, it is better for you to apply for a Freedom Pass as this will allow you to travel for free on trains.
Free travel for under-18s who are not Freedom Pass holders does not extend to trains.
