Transforming Adult Social Care
Introduction
The 'Transforming Social Care Vison' document sets out major changes to the way in which the Council will work.
It forms part of the Council's Personalisation and Self Directed Support programme, which in turn is part of our wider transformation and modernisation agenda.
Here you will be able to find further information on the agenda, our vision, proposed changes and more.
Background
In January 2008 the Government published a key circular on transforming social care, which supports their 2005 'Independence, well-being and choice' green paper.
It outlined proposals for reform and the ways in which changes should be delivered.
At its heart was a shift in the control of services, described as personalisation.
Essentially, what it means is that everyone who receives social care support, regardless of their level of need or how it is funded, should have a choice and control over how that support is delivered.
Our vision
In response to the Government proposals for:
development of a personalised approach to the delivery of adult social care; and
the need to ensure services are relevant, accessible and delivered to a continued high standard.
We have produced our vision document to outline our plans for delivery of these developments.

Timescale
Implementation of the proposed changes will be phased over 18 months and is scheduled to be complete by 1 April 2011.
