The following joint strategy has been produced by Greenwich Council's Adult and Older People's Services and the Greenwich Primary Care Trust.
This strategy envisages a future where disabled people are supported to live independent and fulfilling lives and are respected and included as equal members of society.
The strategy describes the way health and social care services will be commissioned over the coming five years to help realise this ambition.
