Oxleas NHS mental health support

 
Oxleas NHS Foundation Trust Community Mental Health Teams provide:

  • Assessment and shared care
  • Recovery care
Referrals to the assessment and shared care team are people whose needs cannot be met through community counselling.

Assessment and Shared Care Team

This multi-professional team is based at:

The team provides short-term interventions (six months or less) to help service users return to a primary care setting.

Support services are available to assist service users in accessing different aspects of community services, including work and employment, with the goal of promoting recovery.

Recovery Team

This team provides an enhanced service to service users who need longer term interventions (usually up to two years) and offers a high level of support with the emphasis on recovery, and a return to employment, education, or voluntary work.

Drug and alcohol assessment and care management for substance misusers in need of rehabilitation is managed from within this team.

Drop-in services

Both of the above services provide a walk in service improving access for service users.

General Practitioner (GPs) surgeries have proactive Community Psychiatric Nursing engagement and support for service users who are discharged back into their care.

This will ensure that GPs have prompt access to the above teams for their patient if a greater level of intervention is required again.

Greenwich Mind provides a range of social groups and drop-in to support people with mental health needs in Greenwich.
