The care approach ensures that care is targeted directly at the people who most need it. Under the Care Programme Approach (CPA), providers work together to create a programme of care which includes ongoing treatment and rehabilitation, including decisions about suitable accommodation, help with benefits, employment, occupational therapy and leisure activity.
Care plan
If the person requiring attention has been assessed by a mental health worker and, as a result, receives support from community services they will be placed under a CPA. This ensures that a care plan and package is in place when the person leaves assessment or hospital.
Types of support
Support will be provided by a range of providers such as community psychiatric nurses, social services care managers and support workers, housing support workers, psychiatrists, occupational therapists, psychologists and voluntary organisation workers.
A care-ordinator is appointed to be responsible for overseeing the care plan, making sure all planned services are provided and reviewing individual needs.
