What services are on offer?
Once we have worked out what you need and matched it against the eligibility criteria, we can provide the right services for your situation. This may be one or more of the following:
Offer you advice and information to help you arrange support or help for yourself from a range of organisations.
Arrange services, such as care in your own home, day care, meals on wheels, residential or nursing care.
Refer you on to other specialist services across the statutory, independent and private sector. These could include, Community Sensory Services Team, Physical Disability Team, Age Concern, Carer’s Centre, Advocacy Scheme, Welfare Benefits etc.
What about those who care for me?
Carers of individuals meeting the eligibility criteria, will automatically be offerred a Carer’s Assessment, either jointly with you or separately, dependent on the carer’s preference. See

What happens if I can’t return home?
If you need a residential or nursing care home, you will be referred to the Home Finding Team, based at the Queen Elizabeth Hospital. The Team will provide you and your carers with information, support and guidance in the choice of a care home.
