When you come out of hospital, you may need some support. Whether it turns out you'll need full-time care, or a few weeks taking it easy, we can help.
Assessing your needs
On admission to hospital, your nurse will complete a Patient Assessment with you. This will cover all your needs. If you feel that you will need help when you leave hopsital, please advise the nurse completing the assessment.
If you are already receiving help from Social Services, the nurse will refer you to the appropriate Care Management Team for a review of your care needs.
If you are not currently receiving services, a member of the Care Management Team will come and complete a Community Care Assessment with you on the ward. This will include gathering information from everyone involved in your care.
What happens if English is not my first language?
