You can use the forms on this page to request information, and also to make a referral to a relevant drugs and alcohol treatment facility.

Please note that the referral form is for download only, and requires the signature of the person being referred.

Online information form

You can request general information about alcohol and drug treatment and support options in the borough by using our online Information Request form. Just open the form, fill in the relevant fields to request information, and submit the form straight to our Drug and Alcohol Action Team.

Order publicity materials

If you are a practitioner, you can order free drug and alcohol awareness publicity materials such as posters and leaflets.
