Assessing your needs
Your assessment
There are two parts to the assessment. Throughout the process, our staff at the Directorate for Adults and Older People will:
treat you with honesty and respect
work with you and involve you in making any decisions
treat you fairly on the basis of your needs.
Step one: the initial contact
You and a care worker will work out what your needs are in an initial assessment. This step can be done by staff at a Greenwich social services district office, your GP, staff at your housing estate office, or by your sheltered housing manager, staff on your hospital ward or health staff in the community.
Step two: the community care assessment
If it seems you are likely to meet the
eligibility criteria for social services
, a more comprehensive community care assessment will be organised.
Your requirements are then matched against a set of care guidelines. These make sure that our services are allocated fairly and support the people who need them most.
