After the assessment
The outcome of your assessment for care will be written into a care plan and your social worker will do one or more of the following, depending on your care requirements:
Enable you to arrange support or help for yourself from a range of local organisations
Arrange
care in your home
, including services such as meals on wheels
Arrange
care outside the home
, for example at a day centre or in a nursing home
Refer you to other services for specialised assessment
