How to make an appointment
Registrations are made on appointment, which may be made on your behalf by the Hospital/Maternity Bereavement Officer. Please contact the office on 020 8921 5016 if you would prefer to make your own appointment or if you wish to have advice or ask any questions prior to your visit.
If the birth and/or death occurred at another hospital, but you would prefer to attend this office to register please telephone us for advice. This procedure is called 'registration by declaration' and could mean that the issue of legal documents that would allow a funeral to take place may be delayed. As the procedure involves the electronic transfer of information, you would not be able to sign the register - you may wish to consider whether this is important for you.
What does it cost?
The registration of a stillbirth or neo natal death is free of charge. The registrar will provide you with a certificate for the burial or cremation of your child(ren) free of charge, but all other certificates are provided on payment of a fee.
