3. Fill in the application forms
Complete the application form. You can download this from the
The Department for Culture, Media and Sport website

contact us
and we will post one to you.
Complete a ‘Declaration for a Club Premises Certificate’, which you can also find on the The

.
4. Submit your application
Post the forms to us (see

for the address), with the following documentation:
A cheque for the application fee (payable to Greenwich Council) – see the
Licensing Act 2003 Fee Schedule

A plan of the premises – see the

Post a copy of your application forms and premises plan to all the
responsible authorities
